Referrals:

Appointments generally require a referral from a primary care provider (PCP) or another practitioner. Rarely, if the patient does not have a PCP, a one-time consultation may be be scheduled and UVMG will refer the patient to a PCP.

UVMG also welcomes national and international referrals. In the past we have received referrals from the Virgin Islands, Greece, Croatia, Turkey, Hungary and Czechoslovakia to name a few.

Insurance:

Please remember to bring your insurance card and check with your insurance carrier to determine your benefits. Payment for your services including copays, deductibles and self-pay patients will be collected at the time of your appointment.

Patients who receive refills from UVMG are required to be seen at least once a year. Patients are to call five days prior to their last dose. If a patient calls after their prescription has run out, they will only be given enough to get them to their next scheduled follow-up appointment.

Transcriptions:

Please expect a 24 to 48 hour turnaround for consultation reports.
